Brensocatib is indicated for the treatment of non-cystic fibrosis bronchiectasis in people aged twelve years of age and older.
It inhibits, dipeptyl peptide 1 inhibitor (DPP-1) reducing neutrophil serine protease activity during neutrophil development in the bone marrow.
Neutrophils are the core airway inflammatory cell in bronchiectasis.

Brensocatib treatment leads to a reduction in the incidence of exacerbations in patients with bronchiectasis( 20%).
Because DPP-1 as a pivotal role in activation of neutrophil siring proteases, its inhibition is predicted to reduce the production of active neutrophil elastase and other proteases during neutrophil differentiation in the bone marrow.
Lower levels of neutrophil elastase and other neutrophil proteases are noted in sputum of participants who receive brensocatib.
