Cobalt toxicity is a rare condition that occurs from excessive exposure to the metal, typically through occupational inhalation or wear from metal-on-metal hip replacements.
It causes systemic damage, leading to neurological issues-hearing loss, vision loss, neuropathy, heart failure (cardiomyopathy), and hypothyroidism.
Primary Causes
Hip Implants: Wear and tear from “metal-on-metal” artificial hip implants can release cobalt and chromium particles into the body.
Occupational Hazard: Inhalation of cobalt dust or fumes in industries like mining, hard metal processing, and battery manufacturing.
Symptoms Cobalt poisoning affects multiple organ systems, with symptoms usually falling into three distinct categories:
Neurological & Ocular: Peripheral neuropathy, cognitive decline, tinnitus, hearing loss, and optic nerve damage.
Cardiovascular: Cardiomyopathy (enlarged, weakened heart), which can lead to congestive heart failure and shortness of breath.
Endocrine: Hypothyroidism (underactive thyroid), causing fatigue and weight gain.
If cobalt toxicity is suspected—particularly after a hip replacement or due to occupational exposure—doctors typically measure serum or whole-blood cobalt levels.
Cobalt toxicity (arthroprosthetic cobaltism) is a recognized but relatively uncommon complication, and it’s almost entirely linked to metal-on-metal (MoM) hip implants— not the metal-on-plastic or ceramic implants used in most hip replacements today.
Metal-on-metal implants themselves were a minority of hip replacements even at their peak (roughly 1 in 3 hip implants in the US around 2008, dropping sharply after safety concerns emerged around 2010-2012).
Among people with MoM implants, studies suggest elevated blood cobalt levels are fairly common (up to 30-50% in some cohorts), but symptomatic systemic toxicity is much rarer— estimates range from under 1% to a few percent of MoM implant recipients, depending on the study and implant design.
Certain designs especially large-head MoM and some hip resurfacing systems, notably the DePuy ASR, which was recalled in 2010 had notably higher failure and toxicity rates than others.

Cobalt toxicity can cause:
Local tissue reactions (pseudotumors, tissue necrosis) Systemic symptoms: neurological (hearing/vision changes, cognitive issues, peripheral neuropathy), cardiac (cardiomyopathy), and thyroid effects: these systemic cases are rare.
