It is used for chronic weight management in adults and children suffering from rare genetic obesity disorders or acquired hypothalamic obesity by restoring appetite control, reducing hunger, and increasing resting energy expenditure.
It is specifically approved for patients with:Acquired Hypothalamic Obesity (HO): For adults and pediatric patients 4 years of age and older suffering from obesity due to hypothalamic injury, tumors, or lesions.
Rare Genetic Conditions: For adults and children 2 years of age and older with obesity due to Bardet-Biedl syndrome (BBS) or genetic deficiencies in POMC, PCSK1, or LEPR.
Unlike standard GLP-1 weight loss drugs, setmelanotide acts as a melanocortin-4 receptor (MC4R) agonist.
It directly activates the pathways in the brain (the hypothalamus) that regulate hunger, satiety, and energy expenditure, effectively bypassing the biological damage or genetic defects that cause unrelenting hunger.
The medication is administered via subcutaneous injection once a day, typically right when the patient wakes up.
Starting Dose: Usually begins at 0.5 mg to 2.0 mg per day depending on age and indication.
Maintenance Dose: Can be gradually titrated up to 3.0 mg per day.
Setmelanotide is not a general weight loss drug and does not treat the underlying hereditary defects causing the obesity.
Side effects include:Skin Hyperpigmentation: Darkening of the skin, moles, and development of new moles are common.
