Nerandomilast (marketed as JASCAYD®) is an oral, twice-daily phosphodiesterase 4B (PDE4B) inhibitor approved to treat Idiopathic Pulmonary Fibrosis (IPF) and Progressive Pulmonary Fibrosis (PPF) in adults.
It slows lung function decline as measured by forced vital capacity – FVC.
Mechanism: As a PDE4B inhibitor, it offers antifibrotic and anti-inflammatory effects.
Phase III FIBRONEER trials-significantly slows FVC decline in patients, whether used as monotherapy or alongside existing therapies like nintedanib or pirfenidone.
Survival Benefit: Data indicates a potential 32% reduction in the risk of all-cause mortality compared to placebo.
Dosage & Administration: Available as oral tablets, often taken twice daily. It is recommended to use the 18mg dose if taken with pirfenidone.
Safety Profile: Generally well-tolerated, with side effects usually relating to PDE4 inhibition (gastrointestinal).
